Dear Parents/Guardians,

Since March of 2009, Optimus Health Care, Inc. has been providing quality medical, dental and behavioral health services to children at the schools in which we operate School Based Health Centers. Optimus’ School Based Health Center providers do not replace your child’s pediatrician. Rather, they work collaboratively with your child’s pediatrician to provide care to your child during school hours.

School Based Health Centers have been providing care to students in schools for over 25 years and serve as a safety net to improve the health of schoolchildren, reduce absenteeism and increase academic success.

Please complete the consent form to enroll your child in the School Based Health Center. The consent form is valid for two academic years, unless you withdraw your consent. Please read the services provided by our clinical team of primary care providers, medical assistants, dental hygienists, dentists, and social workers.
